29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    48 Main St, Northampton, MA     On 5/7/13, the site foreman directed an employee, who had not previously performed or been trained in the hazards of the task, to place a pry bar beneath a metal road plate that was being wedged upward by an excavator bucket.  The employee was injured when the excavator bucket moved sideways causing the pry bar to strike the employee's leg.
