Springfield, MA 01109: On or about November 29, 2023, the employer did not perform frequent and regular job site inspections by a competent person to identify and correct hazards to which their employees were exposed. a) The employer failed to discover uncapped rebar. b) The employer failed to discover the lack of eye protection use by employees.
