29 CFR 1926.602(b)(3): The safety requirements, ratios, or limitations applicable to machines or attachment usage covered in Power Crane and Shovel Associations Standards No. 1 and No. 2 of 1968, and No. 3 of 1969, were not complied with: PCSA Standard No.3 Section 7.24. The hoist rating at specified radii and vertical distance of dipper pivot pin for that radai shall not exceed the criteria established in paragraph 7.23A,B and C: a) North west Area: The Caterpillar 315B excavator was overloader beyond its capacity when it was used to lift the Caterpillar 257B skid steer loader out of the basement. The excavator tipped over, on or about 5/13/2013.
