29 CFR 1910.95(i)(2)(i): The employer did not ensure that hearing protectors are worn by an employee who is required by 29 CFR 1910.95(b)(1) to wear personal protective equipment: (a) B- Side - On or about May 30, 2013 the Operator on the B-Side was exposed to continuous noise at 133.6% of the permissible daily noise (eight hour time weighted average sound level of 90 decibels) or an equivalent sound level of approximately 92 decibels during the 304 minutes sampling period on May 30, 2013. Exposure calculations included a zero increment for the time not sampled.
