(d)  B-side - On or about May 9, 2013 walking and equipment surfaces had quarter of an inch to 2 inches of cotton lint accumulation.  Cotton lint is an explosive class II dust.      (e) Old Prep Building - On or about May 1, 2013 valve on steam line had defective gasket which caused water to accumulate on floor.
