OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:     (a)  Old Prep Room - On or about May 1, 2013 employee stood on a pallet elevated by a Nissan forklift to connect bag of clay to hoist. Employee was exposed to falling from the pallet. Employee was not protected from falling by a guardrail system or personal fall arrest system. The manufacturer of the forklift states the lift truck is not designed for raising or transporting people.

29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:        (a)  Old Prep Room - On or about May 1, 2013 through May 9, 2013 cotton lint accumulation on surfaces on 2nd and 3rd floors ranged in a height from a quarter of an inch to 3 inches.  There were piles of a combination of cotton lint and cotton meats on those floors were up to 3 feet high.           (b)  Old Prep Room - On or about May 1, 2013 through May 9, 2013 there was a pile of cotton meats over five feet high on the 1st floor due to mechanical equipment failures or malfunctions on the 3rd floor.     (c)  Huller Room - On or about May 9, 2013 walking and equipment surfaces had quarter of an inch to over a foot of cotton lint accumulation.       (d)  B-side - On or about May 9, 2013 walking and equipment surfaces had quarter of an inch to 2 inches of cotton lint accumulation.  Cotton lint is an explosive class II dust.      (e) Old Prep Building - On or about May 1, 2013 valve on steam line had defective gasket which caused water to accumulate on floor.

Multiple boards were laid over a hole in a section on the west side of the floor that was severely decayed. The boards were not secured in place to prevent dispalcement.
