29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):     On or about May 15, 2013 the employer did not ensure the rough terrain fork truck operator was certified to operate the lift, exposing the employee to the hazards associated with operating a rough terrain fork truck.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

General Masonry Construction, Inc., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.451(g)(4)(i), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312519747 , CITATION NUMBER 1, ITEM NUMBER 4 AND WAS AFFIRMED AS A FINAL ORDER ON November 10, 2008 WITH RESPECT TO A WORKPLACE LOCATED AT 932 CASTLEWOOD DR. DEERFIELD, IL 60015.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
