29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) On March 2, 2026, at the above addressed jobsite, an employee was exposed to struck-by hazards from falling material and debris when working below employees performing masonry work. The employee was not wearing a hard hat. No abatement certification or documentation is required for this item.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level. a) On or about February 21, 2026, at the above addressed jobsite, employees conducting work from a frame scaffold, approximately 12 feet above a lower level, were not provided with guardrails, midrails, cross rails or other means of fall protection, thereby exposing the employees to falls. b) On March 2, 2026, at the above addressed jobsite, employees conducting work from a frame scaffold, 12 feet above a lower level, were not provided with guardrails, midrails, cross rails or other means of fall protection, thereby exposing the employees to falls. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
