29 CFR 1910.133(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there was a hazard from flying objects.  a) Goyal Industries, Inc. - Mansfield, Ohio: On or about May 14, 2013, the employer did not assure that employees who were exposed to flying metal particles while operating equipment such as, but not limited to horizontal band saws and belt sanders, had eye protection that incorporated side protection.

29 CFR 1910.133(a)(3):  The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.  a) Goyal Industries, Inc. - Mansfield, Ohio: On or about May 14, 2013, employees that operated equipment such as, but not limited to horizontal band saws and belt sanders, were exposed to eye injuries.  Prescription lenses did not incorporate eye protection to protect against flying particles created while operating the equipment.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Goyal Industries, Inc. - Mansfield, Ohio: On or about May 14, 2013, employees operating the #1 Mazak CNC model VTC-16C (serial #115015) were exposed to injuries from the machine's rotating parts, pinch points, and/or flying chip hazards.  The machine was not adequately guarded in that employees were not prevented from accessing hazard areas during operation or protected while loading/unloading parts and performing minor tool changes.     b) Goyal Industries, Inc. - Mansfield, Ohio: On or about May 14, 2013, employees operating the #2 Mazak CNC model VTC-16C (serial #118008) were exposed to injuries from the machine's rotating parts, pinch points, and/or flying chip hazards.  The machine was not adequately guarded in that employees were not prevented from accessing hazard areas during operation or protected while loading/unloading parts and performing minor tool changes.     c) Goyal Industries, Inc. - Mansfield, Ohio: On or about May 14, 2013, employees operating the #3 Mazak CNC model VTC-20B (serial #134534) were exposed to injuries from the machine's rotating parts, pinch points, and/or flying chip hazards.  The machine was not adequately guarded in that employees were not prevented from accessing hazard areas during operation or protected while loading/unloading parts and performing minor tool changes.     d) Goyal Industries, Inc. - Mansfield, Ohio: On or about May 13, 2013, employees operating the #4 Mazak CNC model VTC-20B (serial #129879) were exposed to injuries from the machine's rotating parts, pinch points, and/or flying chip hazards.  The machine was not adequately guarded in that employees were not prevented from accessing hazard areas during operation or protected while loading/unloading parts and performing minor tool changes.     e) Goyal Industries, Inc. - Mansfield, Ohio: On or about May 13, 2013, the employer did not assure the Sheldon lathe, located in the maintenance shop area, was guarded to protect employees from flying chips and/or rotating parts of the chuck.   f) Goyal Industires, Inc. - Mansfield, Ohio: On or about May 14, 2013, employees operating the Turn-Pro BS-250S horizontal band saw were exposed to amputation injuries in that the unused portion of the saw blade was not guarded.  g) Goyal Industries, Inc. - Mansfield, Ohio: On or about May 13, 2013, employees operating the Turn-Pro BS250S horizontal band saw (s/n 0607188) were exposed to amputation injuries in that the unused portion of the saw blade was not guarded.  h) Goyal Industries, Inc. - Mansfield, Ohio: On or about May 14, 2013, the employer did not assure that rotating parts and nip-points of the small belt sander were guarded.  Employees that used the sander to debur parts in the cutting/deburring area were not protected from hazards created by the rotating parts.
