Employees working within the welding department were required to wear personal protective equipment to perform welding operations and were not trained on the proper use, wear, and limitations. Failure to provide training on personal protective equipment required for wear exposed employees to potential thermal burns associated with welding operations.

An employee who operated the Hyster 40 Fortis (S/n F010V01842N) did not receive training or an evaluation prior to operating the powered industrial truck. Failure to ensure powered industrial truck operators have been trained and evaluated prior to operations exposed employees to struck-by hazards associated with untrained powered industrial truck operators.

On or about June 9, 2023, the employer did not ensure sufficient access and working space was provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment. Access to service disconnect panels and electrical panels adjacent to the jib crane was impeded by production components and refuse.

On or about June 9, 2023, employees performing production operations within Goyal Industries, Inc. were not effectively trained on hazardous chemicals utilized throughout the production area. Employees were unable to cite where Safety Data Sheets were located nor proper measures in the event of contact with hazardous chemicals. Failure to ensure employees are effectively trained on hazardous chemicals exposed employees to potential adverse health effects associated with exposure to hazardous chemicals, such as hydrocarbons contained in lubricating oils.
