29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a. Some authorized employees were not aware of pneumatic energy and isolation devices on the equipment (Versaline) they operated and locked out. b. Some authorized employees were not trained on specific operating procedures, including how to read them and what were the procedures required. Specific procedures were not always used for training authorized employees. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: a. Machines were not always tested after lockout (all employees were not aware of testing requirements or how to read specific procedures for testing). Abatement certification and abatement documentation is required for this violation.
