(a) Screw Machine Specialties, 544 Cleveland Ave., Fruita, CO 81521; On and before May 15, 2013, the exposing employer failed to establish and implement an energy control program consisting of machine specific procedures, employee training, and periodic inspections. Condition exposed employees performing servicing or maintenance to the hazard of unexpected energizing, startup or release of energy from machinery.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. (a) Screw Machine Specialties, 544 Cleveland Ave., Fruita, CO 81521; On and before May 15, 2013, the exposing employer did not provide a suitable method of machine guarding to protect employees from ingoing nip points and rotating parts of five Brown and Sharpe, 1.5 inch capacity, Single Spindle Screw Machine at their Shop in Fruita, CO. Condition exposed employees to amputation and nip point hazards as the machine is in operation. (b) Screw Machine Specialties, 544 Clevaland Ave., Fruita, CO 81521; On and before May 15, 2013, the exposing employer failed to provide a suitable method of machine guarding to protect employees from ingoing nip points and rotating parts for four Armer Multi Spindle Screw Machines at their shop in Fruita, CO. Condition exposed employees to amputation and nip point hazards as the machine is in operation. (c) Screw Machine Specialties, 544 Clevaland Ave., Fruita, CO 81521; On and before May 15, 2013, the exposing employer failed to provide a suitable method of machine guarding to protect employees from ingoing nip points and rotating parts for a Brown and Sharp, 0.5" capacity, Single Spindle Screw Machine at their shop in Fruita, CO. Condition exposed employees to amputation and nip point hazards as the machine is in operation. (d) Screw Machine Specialties, 544 Cleveland Ave., Fruita, CO 81521; On and before May 15, 2013, the exposing employer failed to provide a suitable method of machine guarding to protect employees from the rotating portion and possible flying sparks and chips on a Clausing Lathe, Serial Number 603314, Machine Number 0768. Condition exposed employees to the hazard of rotating parts or sparks as the machine is in use. (e) Screw Machine Specialties, 544 Cleveland Ave., Fruita, CO 81521; On and before May 15, 2013, the expsoing employer failed to provide a suitable method of machine guarding to protect employees from the rotating portion of Single Spindle Davenport Screw Machines. Condition exposed employees to the hazard of rotating parts and ingoing nip points.
