(c) Screw Machine Specialties, 544 Clevaland Ave., Fruita, CO 81521; On and before May 15, 2013, the exposing employer failed to provide a suitable method of machine guarding to protect employees from ingoing nip points and rotating parts for a Brown and Sharp, 0.5" capacity, Single Spindle Screw Machine at their shop in Fruita, CO.  Condition exposed employees to amputation and nip point hazards as the machine is in operation.
