On 7/23/13 one employee operating a lathe was exposed to noise at a dose in excess of the fifty percent dose Action Level.  The employee was exposed to noise at a dose of 70.7%.  This is 1.4 times the Action Level.  The employer had not implemented a hearing conservation program.  This condition exposed the employee to a hazardous noise dose.    (b) Screw Machine Specialties Inc, dba Walker Products Inc at 544 E Cleveland Avenue, Grand Junction, CO: On and preceding 7/23/13, Screw Machine Specialties Inc  not administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, where employee noise exposures equaled or exceeded an 8 hour time weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response), or equivalently, a dose of fifty percent.  On 7/23/13 one employee operating a lathe was exposed to noise at a dose in excess of the fifty percent dose Action Level.  The employee was exposed to noise at a dose of 127.2%.  This is 2.5 times the Action Level.  The employer had not implemented a hearing conservation program.  This condition exposed the employee to a hazardous noise dose.

One employee performed material handling tasks, moving bundles of twelve foot long aluminum or stainless steel rods or moving large wooden boxes on pallets.  Bundles of rods weighed up to approximately 100 pounds.  The employer did not ensure that the employee wore protective footwear.  This condition exposed the employee to a foot crushing hazard.
