29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: Job site located at 16306 R Street, Omaha, NE: Employees were observed conducting construction activities (installing roofing shingles) on a residential structure. The employees were exposed falls of 10 feet or more without using any form of fall protection such as a personal fall arrest system or monitoring system. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder(s) was/were not secured at its top to a rigid support, and/or a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder: Job site located at 16306 R Street, Omaha, NE: An extension ladder used to access a roof surface didnt have the side rails extended at least 3 feet above the upper landing surface (roof). The ladder was used by six employees and the owner to access a residential roof that was being re-shingled. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
