29 CFR 1926.20(b): Accident prevention responsibilities.    29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    a.     On the site where the employer was removing and replacing  electrical heaters and electrical systems, there was no competent person on site to identify hazards relating to falls, and electrical thereby exposing employees to a trip fall hazard.

29 CFR 1926.403(b)(1): Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations:    a.     In the Social Hall, there were energized electrical outlet boxes with  exposed wiring hanging from the ceining, thereby exposing employees to a contact hazard.

29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a.     In Building 4 where employees were performing carpentry work, the electrical outlet covers were removed exposing live energized parts when employees plugged in portable power tools thereby exposing employees to a contact hazard.  b.     In the Social Hall where electrical equipment was removed, the face plates were  removed and not replaced where employees plugged in portable power tools thereby exposing employees to contact hazard.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers.    a.      In Building 4 where an employee was removing electrical heaters and electrical outlets, there were floor holes 9 inches to 15 inches deep which were not cover thereby exposing employees to a trip/fall hazard.    b.      In the Social Hall where an employee was removing electrical equipment, there were floor holes which were not covered thereby exposing employees to a trip/fall hazard.
