29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: (a) On or about 5/15/2013, on the south wall of the facility, a door that was marked with an illuminated exit sign was bolted shut, preventing employees from being able to open the door. Employees were exposed to burns and/or smoke inhalation hazards.

29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wears eye protection that incorporates the prescription in its design, or wears eye protection that can be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses: (a) On or about 5/15/2013, throughout the facility, the employer did not provide prescription safety glasses to those employees who wore prescription glasses nor did he provide eye protection that could be worn over the prescription glasses. Employees were exposed to hazards of eye injuries.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: (a) On or about 5/15/2013, in the production area, an Amerex fire extinguisher was not identified so that it could be easily located in the event of a fire. The employees were exposed to hazards of burns and/or smoke inhalation. (b) On or about 5/15/2013, in the production area, a Badger fire extinguisher was not mounted on the wall, but had been placed on top of an electrical panel box. Employees were exposed to hazards of burns and/or smoke inhalation.
