29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not used: (a) Location: Route 16, Lot 5211-2-2-1-R2-1, Barrigada, GU 96913 This violation was discovered on 5/21/2013 where, hand protection was not used when employee mixed and applied QUIKRETE Concrete Bonding Adhesive on piping structure bonded to concrete floor. Employees were exposed to bonding agent contacting the skin.
