29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: a) In the facility, on or about October 21, 2013 - Where the Unit 20 Shell and Tube Reactor was provided with overpressure protection by system design in lieu of a pressure relief device, the employer did not document that the equipment complied with RAGAGEP, such as ASME Boiler & Pressure Vessel Code Section VIII Code Case 2211 for overpressure protection by system design. b) In the facility, on or about October 21, 2013 - Where the Unit 21 East Spiral Reactor was provided with overpressure protection by system design in lieu of a pressure relief device, the employer did not document that the equipment complied with RAGAGEP, such as ASME Boiler & Pressure Vessel Code Section VIII Code Case 2211 for overpressure protection by system design.
