29 CFR 1910.27(c)(4):     The employer did not install fixed ladders with at least 7 inches of clearence from the centerline of the rung to the nearest permanent object in the back of the ladder.     a)  Establishment - 2 Crossover Bridges with 1 adjacent to the Gemini conveyor system and one adjacet to the Rack Loader:    Employees were exposed to falls as each Crossover Bridge had only 2 inches of clearance from the centerline of the top rung to a retrofitted metal shield behind the rung.                                           Violation observed on or about 5/20/2013.
