29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports: JOB SITE: 200 Harding Avenue, Maryland Heights, MO 63043, at this work site, a worker was observed on the lower tier (working level) of a tubular welded scaffold, set atop of the church steeple, approximately 21 feet 3 inches, from peak of the roof to ground level, actively painting the steeple and the working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports. The worker was exposed to a fall hazard.

29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling: JOB SITE: 200 Harding Avenue, Maryland Heights, MO 63043, at this work site, a worker was observed working on the front edge of the platform of a tubular welded scaffold approximately 21 feet 3 inches from peak of the roof to ground level, and was greater than 14 inches from the face of the work. The employer failed to install a guardrail system on the front edge and PFAS was not used.

The worker was exposed to a fall hazard.
