29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid and capable of supporting the loaded scaffold without settling or displacement: a. Site- Employees working from a Skyjack SJ 9250AL scissor lift were not protected from scaffold tipping and collapse hazards where one of the outriggers supporting the weight of the lift was set upon a wooden pallet. On or about 5/14/2013 two employees sustained injuries after the scissor lift they were working from moved unintentionally when retracting the front outriggers and the outrigger set on the wood pallet retracted faster than the other outrigger. The scissor lift lurched to one side and sheets of sheetrock loaded on the lift fell on top of the employees in the lift.

Site- Employees working from a Skyjack SJ 9250AL scissor lift were not protected from scaffold tipping and collapse hazards where the lift was loaded beyond its rated capacity. On or about 5/14/2013 two employees sustained injuries after the scissor lift they were working from moved unintentionally when retracting the front outriggers and the outrigger on the right side, the side of the lifter where the load was set, retracted faster than the other outrigger. The scissor lift lurched to the right side and sheets of sheetrock loaded on the lift fell on top of the employees in the lift.

Site- Employees operating and working from a Skyjack SJ 9250AL scissor lift were not trained by the employer to recognize scaffold hazards associated with the proper set up, operation, and loading of the lift and the procedures to control and minimize the hazards. On or about 5/14/2013 two employees sustained injuries while working from a scissor lift when the load on the lift fell on top of them.
