29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Job site, 1001 Ohio River Boulevard, Avalon, PA: On or about May 22, 2013, during santarty line installation activities, the employer did not ensure that appropriate cave in protection was provided and used along a ledge built into the face of a hillside excavation. The excavation in total was 30 feet deep, with a ledge about halfway up on the hillside excavation, and employees were working next to a near vertical wall determined to be approximately 6 1/2 to 7 feet in height, and soils on the slopes above them were determined to not be at a correct angle.
