While the cover is not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings:  (a) Chiller Area -  A section of the chiller pit cover was open and was not contantly attended by someone or wasn't protected on all exposed sides by removable standard railings.

Mt. Prospect Road, Des Plaines, IL 60018.
