29 CFR 1926.652(g)(2): Excavations of earth material to a level greater than 2 feet (.61 m) below the bottom of a shield was permitted: A. VTF Excavation, LLC. worksite located near 207 S. Broadway, Columbus Grove, Ohio: On or about May 22, 2013, the employer did not ensure the bottom of the protective manhole boxes were within 2ft from the base of the excavation on the north end of the trench. An employee was exposed to potential wall failure when the bottom of the protective shield was approximately 4.00 feet above the bottom of the excavation.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that was 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: A. VTF Excavation, LLC. worksite located near 207 S. Broadway, Columbus Grove, Ohio: On or about May 22, 2013, the employer did not ensure a safe means of egress such as a ladder was provided to an employee installing sanitary lines in a protected trench which was approximately 16.00 feet deep. VTF Excavation, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.651 (c)(2) which was contained in OSHA inspection number 313781072, citation number 1, item number 1 and was affirmed as a final order on February 18, 2010, with respect to a workplace located at Celina, Ohio

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: A. VTF Excavation, LLC worksite located near 207 S. Broadway, Columbus Grove, Ohio: On or about May 22, 2013, the employer did not ensure an employee was adequately protected from cave-ins while installing a sanitary sewer structure in the north end of the trench. An employee was working within stacked manhole boxes in a trench which was approximately 16.00 feet deep. No cave-in protection was provided on the north end of the manhole box. VTF Excavation, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652 (a)(1), which was contained in OSHA inspection number 313781072, citation number 1, item number 2 and was affirmed as a final order on February 18, 2010 with respect to a workplace located at Celina, Ohio.
