29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):    (a)  VTF Excavation, LLC on or about April 23, 2015, at a trench located on Caroline St. east of Williams St. in Paulding, OH: The employer in control of the jobsite (as prime contractor) failed to ensure employees of a subcontractor installing a by-pass valve in a 7 ft. deep trench were adequately protected from cave-ins after awareness of the hazard was known.     (b)  VTF Excavation, LLC on or about April 23, 2015, at a trench located on Caroline St. west of Williams St. in Paulding, OH: The employer in control of the jobsite (as prime contractor) failed to ensure employees of a subcontractor installing a by-pass valve in a 6.6 ft. deep trench were adequately protected from cave-ins after awareness of the hazard was known.     VTF Excavation, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 908210, citation number 2, item number 2 and was affirmed as a final order on November 20, 2013, with respect to a workplace located at 207 S. Broadway, Columbus Grove, OH.
