Employees are exposed to caught by hazards in the event of an unexpected energization of the rotating dispensing wheel while clearing paper that has become entangled around the dispensing wheel. see FSA for modified AVD

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a)On or about May 30, 2013, the employer did not assure employees were protected from caught in hazards while operating Model 4 Ropak Machines in the splenda room. Employees are exposed to rotating fingers spinning at approximately 3000 rpms while adjusting the dispensing weight of splenda on a knob located approximately 6 inches from the spinning wheel. see FSA for modification of AVD.
