29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) The employer did not conduct an annual periodic review of the energy control procedures for equipment such as but not limited to the Tri-Casting machines. This is to ensure that employees maintain the skills and proficiency to perform adequate energy control to pieces of equipment they service and maintain. Employees were exposed to entanglement and/or crushing hazards from performing mold and trim die changeovers without utilizing lockout/tagout.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6) (a) On or about May 24, 2013, an employee was performing a change-over of the trim die in the Tri-Caster 4A and did isolate the hydraulic and electric energy sources and apply locks. The employee was exposed to crushing and/or entanglement hazards from the energized trim die.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: (a) The employer did not ensure that employees understood the purpose and function of energy control by training them on effective use of lockout/tagout applications. On or about May 24, 2013, an employee was not using lockout/tagout when changing a trim die and his hand was crushed in the Tri-Caster 4A trim die.
