29 CFR 1926.28(a): The employer is responsible for requiring the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees.  a. On the roof of the house where an employee was using an air actuated nail gun to install the roof sheathing, eye protection was not used, thereby exposing the employee to a struck-by hazard.  b. On the second floor below the employee working on the roof, hard hats were not worn to protect employees from objects falling from above, thereby exposing employees to a struck-by hazard.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.  a. At the temporary electrical panel on the south side of the house, the ground prong was missing from the extension cord, thereby exposing employees to a contact hazard.  b. At the three way connector in the rear of the house, a ground prong was missing from an extension cord, thereby exposing employees to a contact hazard.

29 CFR 1926.405(a)(2)(ii)(J): Extension cord sets used with portable electric tools and appliances shall be of three-wire type and shall be designed for hard or extra-hard usage. Flexible cords used with temporary and portable lights shall be designed for hard or extra-hard usage. NOTE: The National Electrical Code, ANSI/NFPA 70, in Article 400, Table 400-4, lists various types of flexible cords, some of which are noted as being designed for hard or extra-hard usage. Examples of these types of flexible cords include hard service cord (types S, ST, SO, STO) and junior hard service cord (types SJ, SJO, SJT, SJTO).   a. In the rear of the house where employees were cutting lumber with a circular saw, the extension cord being used was not designed for hard or extra-hard usage. Additionally the cords outer insulation was frayed and coming off in three areas along it's length, thereby exposing employees to a contact hazard.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.  a. On the roof of the house where an employee was installing the sheathing, fall protection was not used, thereby exposing the employee to a 25 foot fall hazard.  b. On the second floor where employees were cutting lumber next to an open window, the openings were not guarded against falls, thereby exposing employees to a 15 foot fall hazard.  The Framing Carpentry Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 110645, citation number 1, item number 1 and was affirmed as a final order on February 18, 2012 with respect to a workplace located at 261 McCumber Loop, Delaware, Ohio 43015.
