29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about May 24, 2013, on the north side of the commercial building, employees who worked without fall protection were exposed to a fall hazard of approximately 12 feet while engaged in roofing activities.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about May 24, 2013, on the north side, employees engaged in roofing activities on a commercial building utilized an extension ladder that did not extend 3 feet above the roof landing exposing employees to fall hazards.
