Capacity, operation and maintenance instruction plates, tags or decals shall be changed accordingly:     On or about 5/22/13     Location: 519 East 132 Street, Bronx, NY 10454     a) A home made boom type fork extension was placed on the original manufactures forks of the Clark model CGP-50 forklift. The boom extended 4.8 feet longer than the forks.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:     On or about 5/22/13     Location: 519 East 132 Street, Bronx, NY 10454     a) The fuel tank locater pin was missing on the Clark  model CGP-50 forklift.     b) The safety latch was missing on the Clark CGP-50 forklift boom hook.     c) The hook and clevis on the boom of the Clark CGP-50 forklift were not made of alloy steel suitable for the lifting of loads.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:      On or about 5/22/13      Location: 519 East 132 Street Bronx, NY 10454      a) A damaged 100 foot orange extension cord was not removed from service. The cord had areas of damage to the outer insulating jacket that were wrapped in duct tape and areas where the outer insulation was completely missing.     b) A damaged black 240 volt extension cord was not removed from service. The cord had an area of damage near the male plug end that was wrapped in duct tape.
