This violation was discovered on May 30, 2013 at a job site located on the 2nd floor of the Micronesian Mall at 1088 W. Marine Dr. Dededo, GU 96912, where a worker was exposed to a fall hazard while working from a metal scaffold platform that was modified by using a 2 inch by 4 inch wooden form to support the platform.
