29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. (a) Division IV, Inc., 3555 Colorado Blvd., Denver, CO 80205: On and before May 29, 2013, the exposing employer did not ensure that employees utilizing scaffolding were protected from falling to a lower level through adequate fall protective measures, specifically, top and mid rails at the correct height. Condition exposed employees to fall hazards of approximately 18 feet as they performed masonry work.

29 CFR 1926.451(f)(1): Scaffolds and scaffold components shall not be loaded in excess of their maximum intended loads or rated capacities, whichever is less. (a) Division IV, Inc. 3555 Colorado Blvd., Denver, CO 80205; On and before May 29, 2013, the exposing employer allowed a pallate of CMU blocks to be placed on a scaffold causing the scaffold component planking to be loaded in excess of the intended load. Condition exposed employees the hazard of possible scaffold collapse and/or struck by hazards for any employee working below.
