29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  (a) 1785 Beach Blvd in Biloxi, MS - On or about May 23, 2013, an employee was using a 2 by 12 board for a scaffold platform. The actual width was 11.5 inches.

29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface:  (a) 1785 Beach Blvd in Biloxi, MS - On or about May 23, 2013, an employee was standing with one foot on a scaffold platform and the other foot about 36 inches away to another surface on a wall.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) 1785 Beach Blvd in Biloxi, MS - On or about May 23, 2013, an employee was working between one of the scaffolds and the edge of the 2nd floor without any fall protection. The edge of the 2nd floor was 17 feet above the ground.

(a) 1785 Beach Blvd in Biloxi, MS - On or about May 23, 2013, the extension ladder used by employees to access the 2nd floor extended only 6 inches above the upper landing.
