29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.Note: This paragraph does not apply to large area scaffolds where the employer has satisfied the following criteria: the ladder was placed against a structure which was not a part of the scaffold and the scaffold was secured against the sideways. (a) Front of building - On or about June 16, 2020 the employer allowed employees working from the scaffolding to use a step ladder to increase their working level, exposing employees to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. (a) East side of scaffolding - On or about June 16, 2020 The employer allowed employees working from the welded tubular scaffolding to access through the cross bracing, exposing them to falls hazards.
