29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: On or about May 21, 2013, employees to access the roof used the 6-foot, A-frame ladder located in front and to the left corner of the house. Employees needed over 2-feet more to reach the top of the roof and had to use the top step of the ladder to climb onto the roof. Employees were exposed to fall hazards approximately 8-feet 6-inches from the top of the roof to the ground.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: On or about May 21, 2013, the employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees: a) A 100-foot orange extension cord used to power the 7 1/4-inch Makita Circular Saw (model 5007F, serial #1039645Y) had outer insulation damage. b) A 100-foot orange and black extension cord used to power the 7 1/4-inch Worm Drive construction saw had outer insulation damage. c) The 7 1/4-inch Worm Drive construction saw used in the construction process had strain relief broken.
