ADL Design and Construction Company, LLC. had employees exposed to fall hazards on a construction site in Danville, Illinois.  The employer did not have an effective safety and health program with specific work rules, competent person oversight, and employee training to govern employees activities.

ADL Design and Construction Company, LLC. had employees exposed to fall hazards on a construction site in Danville, Illinois. 29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) shall be installed so that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform).     ADL Design and Construction Company, LLC. had employees exposed to fall hazards on a construction site in Danville, Illinois.  The employer did not fully plank or deck each working level between the front uprights and the guardrail supports.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.     ADL Design and Construction Company, LLC. had employees exposed to fall hazards on a construction site in Danville, Illinois.  The employer had employees accessing different working levels on the scaffolding by climbing the scaffold frame.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:   ADL Design and Construction Company, LLC. had employees exposed to fall hazards without fall protection, on a construction site in Danville, Illinois.  The employer had an employee working from a JLG at heights greater than 10 ft. without a body belt and lanyard attached to the boom or basket of the JLG.  The employer did not provide scaffold training.  This training would include:      Nature of any electrical hazards, fall hazards, and falling object hazards in the work area, Correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems used, Proper use of the scaffold, and the proper handling of material on the scaffold,  The maximum intended load and the load-carrying capacities of the scaffold used, Any other pertinent requirements of this subpart, L  Scaffolds.
