29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a)  Alan Kunsman - On or about May 30, 2013, employees were accessing the roof of a residence using an extension ladder that did not extend at least three feet above the upper landing surface nor was it secured to prevent displacement and used with a grasping device.  ALAN KUNSMAN ROOFING & SIDING, INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.1053(b)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312980824, CITATION NUMBER 1, ITEM NUMBER 5, AND WAS AFFIRMED AS A FINAL ORDER ON JULY 2, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT  5060 PA ROUTE 873, SCHNECKSVILLE, PA.
