29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:     a.  Paint/Blasting area and booth: Employees are required to wear a half face tight fitting negative pressure air purifying respirator during painting operations and a supplied air hood while metal abrasive blasting.  An employee was not provided with a medical evaluation prior to the wearing the respirators, as determined on 5/30/13.      Martin Steel, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(e)(1), which was contained in OSHA inspection number 313372633, citation number 1, item number 1(a) and was affirmed as final order on 7/08/10, with respect to a workplace located at 603 16th Street West Huntington, WV 25704.     Abatement Certification Required For This Item.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:      a. Paint/Blasting booth: A blasting hood/respirator used by an employee was stored hanging on a hook in the blasting booth exposed to dust and blasting media with the air supply hose lying in the abrasive dust on the floor.  The respirator was not protected from damage, contamination, or dust, as determined on 5/30/13.      Martin Steel, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(h)(2)(i), which was contained in OSHA inspection number 313372633, citation number 1, item number 1(c) and was affirmed as final order on 7/08/10, with respect to a workplace located at 603 16th Street West Huntington, WV 25704. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. Bay #1 Punch: Employees used a Peddinghaus 225B/20 full revolution punch #FH90 to punch holes in metal sheet.  The punch was not guarded to protect the operator or other employees in the area from the point of operation or flying chips that are being punched out of the metal, as determined on 5/30/13.    b. Bay #3 Ironworker: Employees used a Peddinghaus 110/140 Ironworker #442364660206036 to punch metal sheet.  The punch guard was damaged with a 2-1/2 inch vertical x 1-1/8 inch horizontal opening exposing the operator or employees in the area to the point of operation or flying chips punched out of the metal, as determined on 5/30/13.    c. Oster threading machine: Employees use an Oster 784 XZ-27 Pipe & Bolt machine to thread metal stock.  The rotating chuck at the rear of the machine had protruding jaws and was unguarded, as determined on 5/30/13.    Abatement Certification Required For This Item.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:     a. Prep shop Cincinnati shear:  Employees used a Cincinnati 12' shear to cut metal plate that had a guard 1-3/4" to 2" above the table and within 3" of the point of operation.  The guard would not prevent employees from coming into contact with the hold downs and shear blade during operation of the shear, as determined on 5/30/13.
