29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  (a) Prep Shop, Beam Saw: On October 10, 2019 and times prior, an employee was exposed to occupational noise at 132.9% of the permissible daily noise exposure (eight hour time weighted average of 90 dBA at the 80 dB criterion level) during the 484 minute sampling period on October 10, 2019.  For a full eight hour shift this would be equivalent to an average sound level of 92.0 dBA. The employer did not establish and maintain an audiometric testing program for the employee.  (b) Fabrication/Welding Area: On October 10, 2019 and times prior, an employee was exposed to occupational noise at 108.0% of the permissible daily noise exposure (eight hour time weighted average of 90 dBA at the 80 dB criterion level) during the 480 minute sampling period on October 10, 2019.  For a full eight hour shift this would be equivalent to an average sound level of 90.5 dBA. The employer did not establish and maintain an audiometric testing program for the employee.  (c) Fabrication/Welding Area: On October 10, 2019 and times prior, an employee was exposed to occupational noise at 163.6% of the permissible daily noise exposure (eight hour time weighted average of 90 dBA at the 80 dB criterion level) during the 481 minute sampling period on October 10, 2019.  For a full eight hour shift this would be equivalent to an average sound level of 93.5 dBA. The employer did not establish and maintain an audiometric testing program for the employee.
