The employee was not protected by any guardrail systems and/or other fall protection devices.    IN THE ALTERNATIVE    29 CFR 1910.132(a):  Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    A.)  Permco Contracting, Inc., 20 South Main Street, Union City, PA  16438, on or about May 30, 2013 and times prior thereto:  An employee performing roofing work was exposed to a 25 foot fall hazard to the ground below.  The employee was not protected by a personal fall arrest system (PFAS) and/or other fall protection device.
