Permco Contracting, Inc., 9615 US Route 6, Union City, PA 16438, Rooftop of Sharon Syeds Brides Palace, on or about May 30, 2013, and times prior thereto: Company employees were utilizing Rapid Roof HV Basecoat chemical in a plastic 5 gallon bucket. The plastic bucket was labeled incorrectly in that it was labeled with a Master Guard Wall Kote Exterior Coating plastic container.

The plastic bucket was not labeled with the appropriate hazard warnings in that, the plastic bucket contained the hazard warnings associated with a Master Guard Wall Kote Exterior Coating chemical.
