29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about 31 May 2013, and at times prior thereto, the employer did not establish a lock-out/tag-out program for the employees in the workplace who change out blades for the wood cutting machinery, exposing them to amputation hazards.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.  On or about 31 May 2013, and at times prior thereto, employees would utilize electrical housings that had open knockouts, exposing the employees to an electrical shock hazard of approximately 120 volts. 1) Outside the shop floor office, the electrical housing for the office light switch had an open knockout on top. 2) On the west wall, an electrical housing for a two-receptacle outlet had a missing knockout.

29 CFR 1910.305(b)(2): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy.  On or about 31 May 2013, and at times prior thereto, employees utilized energized 120 volt outlet receptacles that were not dead-front, resulting in the internal wires and terminal screws being visible, exposing the employees to electrical hazards. 1) The energized 120-volt receptacle located along the west side wall was not dead-front and the faceplate was hanging out by the internal wires and terminal screws. A cord was plugged into the outlet at the time of the inspection, which was providing power to the company's wireless router. 2) The energized 120-volt receptacle located on the wall between the employee break room door and the bathroom door had damage to the faceplate at the time of the inspection, with wires visible.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure the electrical outlets are maintained and free of damage.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    On or about 31 May 2013, and at times prior thereto, the employer did not keep the shop floor clean and orderly of excessive sawdust, which had collected on the support structure, on top of the employee breakroom, and in and around the chop saws and various other locations, exposing the employees to a flash fire hazard.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure the shop area is kept free of sawdust.    Artisan's Row Cabinetry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(1), which was contained in OSHA inspection number 314304486, citation number 01, item number 001 and was affirmed as a final order on 1-14-2011, with respect to a workplace located at 2200 S. Church Street, Suite 102, Georgetown, TX 78626.
