29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a. On or about June 1, 2013, on a 6:12 pitch roof, 10 employees performing roofing operations were not protected from an 8 feet 9-inch fall hazard by use of a guardrail system, safety net system or personal fall arrest system.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder:  a. On or about June 1, 2013, on the east side of the building, employees used a 24 foot Louisville extension ladder to access a roof that did not extend 3 feet above the eave nor was secured from slippage, exposing employees to an 8 feet 9-inch fall hazard.    b. On or about June 1, 2013, on the west sdie of the building, exployees used a 24 foot Louisville extension ladder to access a roof that did not extend 3 feet over the eave nor was secured form slippage, exposing employees to an 8 feet 9-inch fall hazard.
