29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  a.  On or about January 21, 2020, at the Lake Crossing Apartments in Gainesville, at Building K, the JCB Model 510-56 Telescopic Handler was used to support scaffold platform that was not specifically designed by the manufacturer for such use, exposing employees to a 30 foot fall hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a.  On or about January 21, 202020, at the Lake Crossing Apartments in Gainesville, on the 6:12 pitch roof of Building K, a roofing employee was exposed to a 30 foot fall hazard while relocating anchor points in that he was not tied off.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.    On or about January 21, 202020, at the Lake Crossing Apartments in Gainesville, at the northwest end of Building K, on the 6:12 pitch second-story roof, the portable aluminum extension ladder used to access the third-story roof from the second-story roof did not extend at least feet above the upper landing surface, exposing 3 employees to a 10 foot fall hazard.
