Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. The Warehouse. a) Bulks of supplies and merchandises were stored on top of storage racks near entrance door with no interlocks creating an overhead hazard when driving power industrial trucks near the racks. On or about 5/29/2013.

The name of the operator. 2. The date of the training. 3. The date of the evaluation; and 3. The identity of the person(s) performing the training or evaluation.
