29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:   a) At the two blow heat recovery accumulator tank platforms, on or about June 13, 2013 and at times prior to, employees used the fixed access ladders that had an unguarded ladderway entrances, resulting in a ten-foot and twenty-foot fall hazard.   b) On the south side of the Recovery Boiler #5, at the 4th floor incinerator platform, on or about June 13, 2013 and at times prior to, employees used the platform where the fixed service access ladder had an unguarded ladderway entrance, resulting in a ten-foot fall hazard.  ABATEMENT CERTIFICATION AND DOCUMENTATION IS REQUIRED.

29 CFR 1910.305(a)(2)(i): Temporary electrical power and lighting installation(s) 600 volts, nominal, or less were used in prohibited manners:   a) In the caustic operations shop office/break area, on or about June 13, 2013 and at times prior to, flexible cords were being used on a permanent basis to provide power to equipment including a ceiling fan and a water cooler, resulting in an electrical hazard.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:   a) At the caustic operations shop, on or about June 13, 2013 and at times prior to, in lieu of installing a fixed receptacle near the lockers, an extension cord, which was plugged into a receptacle in the office/break area was used to power battery chargers that were located at the lockers in an adjacent room, resulting in an electrical hazard.
