29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a)  Operating Rooms - Employees used Isolyser LTS-Plus to solidify liquid in canisters in the operating room.  The employer did not ensure employees wore appropriate eye protection, on or about June 5, 2013.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    (a) Operating Rooms - Employees used Isolyser LTS-Plus to solidify liquid in canisters in the operating room.  The employer did not ensure employees received adequate training on the hazards of the chemical, on or about June 4, 2013.      The Penn State Milton S. Hershey Medical Center was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(1), which was contained in OSHA inspection number 315886838, citation number 1, item number 3(b) and was affirmed as a final order on 3/6/2012, with respect to a workplace located at 500 University Drive, Hershey Pennsylvania.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
