29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  a) West side of structure, on sheathed roof areas; On or about June 5, 2013 and at times prior thereto, the employer did not provide any, adequate and/or effective fall protection training for two employees performing framing/sheathing work at elevated areas of a residential structure. One employee was wearing a harness incorrectily and was not connected to the horizontal rope lifeline located on the roof, which was also anchored incorrectly. The other employee was intially connected to a horizontal rope lifeline that was anchored correctly but ended up attaching the anchor end of the lifeline to his harness and tied the other end of the lifeline to the structure.  Note: Abatement certification and documentation IS required for this item.   Guate Construction was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 465358, citation number 1, item number 2b and was affirmed as a final order on October 2, 2012, with respect to a workplace located at 1625 W.
