29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. 57 Locust Ln, East Rutherford, NJ 07073: An employees engaged in roofing work was exposed to a fall hazard of approximately 30 feet while working without a personal fall arrest system or other means of fall protection, on or about 06/05/2013.  Mike's All Seasons Roofing & Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 312864952, citation number 2, item number 1 and was affirmed as a final order on 9/16/2011, with respect to a workplace located at 431 Woodridge Ave, Wood Ridge, NJ 07075.  Mike's All Seasons Roofing & Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 312859655, citation number 1, item number 1 and was affirmed as a final order on 9/23/2009 with respect to a workplace located at 735 Lewandowski Street, Lyndhurst, NJ 07071.
