29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    a) Front  wall: Employees were exposed to a fall hazard of approximately 17 feet while working from a pump jack scaffold platform that did not have a complete guardrail system or other acceptable means of fall protection in place, on or about 5/7/14.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:        a) Throughout the site: Employees that had not been trained in the recognition, control and elimination of fall hazards were exposed to fall hazards while working from a pump jack scaffold platform and roof surfaces, on or about 5/7/14.

The Mike's All Seasons Roofing & Siding  was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 910111, citation number 01, item number 01 and was affirmed as a final order on 7/26/13, with respect to a workplace located at 56 Locust Ln., East Rutherford NJ.
