29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible engineering controls were not utilized: a) Extruder/Production Area: CMD winder machine operators were exposed to continuous noise levels at 139% and 156.2% of the 8-hour time weighted average permissible exposure level of 90 decibels. The equivalent dBA levels of 139% and 156.2 % are approximately 92.37 dBA and 93.21 dBA respectively. The samples were performed for 423 and 465 minutes during the morning shift on June 11th, 2013. Zero exposure was assumed for the unsampled period of time. The employer did not utilize engineering control, on or about 06/11/13.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) Extruder/Production Area: CMD winder machine operators were exposed to continuous noise levels at 128.2%, 139 %, and 156.2 of the 8-hour time weighted average sound level of 85 decibels. The equivalent dBA levels of 128.2%, 139 %, and 156.2% are approximately 91.79 dBA, 92.37 dBA, and 93.21 dBA respectively. The samples were performed for 444, 423, and 465 minutes during the morning shift on June 11th, 2013. Zero exposure was assumed for the unsampled period of time. The employer did not administer a continuing, effective hearing conservation program, on or about 06/11/13.
